Registration Forms
The following documents must be submitted for registration and enrollment:
- Enrollment
- About Your Child information
- Waiver
- Emergency information
- Authorized Persons pick-up
- Copy of Birth Certificate
- Copy of updated Shot Record - annual PPD reading required
- Copy of updated Physical - annual physical required
All information should be complete and current and must be updated when any change occurs.
